A three-year-old boy died of septic shock at a West Coast hospital after medical errors occurred. An investigation found that an on-call doctor and a nurse in charge of the children's ward missed critical opportunities to save his life. The Health and Disability Commissioner has since written to Health NZ regarding the incident.
