Grandmother says mental health system failed teenager who died by suicide

A grandmother has claimed that inadequate mental healthcare failed her grandson, who died by suicide in February 2023. The teenager had been struggling with substance abuse while living with his grandmother. Coroner Mary-Anne Borrowdale has ruled the cause of death.
