A coroner has ruled that inadequate safety measures on Wellington's waterfront may have contributed to the drowning of 30-year-old Sandy Calkin. The report suggests the death could have been avoided if the Wellington City Council had investigated previous accidents and followed expert advice. Calkin's body was found in the harbour after he fell from Queens Wharf.
