The Health and Disability Commission found that communication and handover failures delayed treatment for a man at Dunedin Hospital in 2021. After suffering a fall, the man's leg was amputated due to delayed diagnosis of blocked blood flow, and he died shortly after. Health New Zealand Southern has been asked to apologise to the family.

On 19 March, an orthopaedic senior medical officer suspected the man's left leg had a sudden, severe drop in blood flow and required vascular review.
